1. Membership fee payment is due once a year, and membership shall be valid for twelve months (one year) counting from the second month after the application is filed. Ex:if the application for membership is submitted in December 2006, the membership will be valid starting January 2007 until December 2007 and so forth. If any changes are made, please refer to the announcements of StarJerry.
2. Membership application fee is US$8 (to be collected upon joining membership), and annual membership fee is NT$800 per year.

* Online registration on the official website:
Please choose the ?Online registration? function on the official website, read and agree to follow StarJerry’s membership regulations, and fill out the online application form. For the membership fee, choose to pay online through credit card or scan your wire transfer receipt and upload the file.
* Mail:
Please download the application form from the official website. Fill out the application form and mail it with a copy of your wire transfer receipt to ?P.O. Box 87-359 Taipei, Taipei City 10599, Taiwan (R.O.C) ? for your membership fee to be processed.
5. Notes:
1. Please keep your application form and original wire transfer receipt for verification in the future.
2. We will not accept cash or checks through mail.
3. Under all circumstances, the bank’s transaction fee should be paid by the applicant.
4. After confirmation of your application form and remittance data (this process will require 2-4 weeks), you will be notified of your membership No. through e-mail.
5. To prevent processing inconvenience, please do not apply online and through mail at the same time.
6. If you are under 20 years old, before you apply to become a member, your parent (or guardian) must read, understand and agree to all contents and modifications of this agreement.
